The Impact You Will Have
The social worker position is an important role to assist our members and their families with gaps in care. The position exists to ensure our members are supported and given appropriate resources depending on their individual needs.
Your Responsibilities
- Responds to member, provider, customer service, company and external partner referrals and/or questions. Provides referrals, education, and support coordination to appropriate community resources and tracks outcomes of those referrals.
- Evaluates members needs in order to address social determinants of health (SDoH) concerns such as transportation/housing/food insecurity and provides assistance to address gaps/barriers. Assist with quality improvement projects and population health initiatives (e.g. Stars and HEDIS).
- Provide caregiver support/resources and referrals. Serves as a resource to members and their families (e.g. navigating services in the health care system, needs assistance with home modifications/care at home, needs information about Medicare/Medicaid).
- Screens members for social/emotional factors such as depression, isolation and anxiety. Gathers accurate and comprehensive information required for benefit and/or member management, as required including the administration of screening tools in areas such as mental health, substance use, or social determinants of health needs. Facilitates referrals to Care Management Programs as appropriate.
- Collaborates and coordinates with team members to facilitate day to day functions and enhance the overall operation of the department.
- Serves as a subject matter expert to team members and BCBSMN, providing expertise and experiences to enhance care models, processes and procedures to more effectively address the holistic health and wellbeing needs of members and families.
- Attends weekly ICT (Interdisciplinary Care Team) rounds and provides feedback, if appropriate.
- Maintains a caseload and documents in a plan of care, if appropriate.
Required Skills and Experience
- Licensed Social Worker (LSW) with no restrictions and 3 years relevant experience.
- 1 year of county or hospital social work experience.
- Medicare and/or Public Programs experience.
- Flexibility to work varied hours.
- Demonstrates strong empathy and relationship building skills.
- Strong acumen in communication skills including listening, speaking and writing skills.
- Strong computer application skills.
- High school diploma (or equivalency) and legal authorization to work in the U.S.
Preferred Skills and Experience
- Medical call center or managed care experience.
- Process and project management experience.
- Licensed Independent Clinical Social Worker (LICSW)
Compensation and Benefits:
Pay Range: $67,200.00 - $89,100.00 - $111,000.00 Annual
Pay is based on several factors which vary based on position, including skills, ability, and knowledge the selected individual is bringing to the specific job.
